How To Become A New Practice Member
Step 1: Schedule an appointment
To get started, schedule your first visit by calling or texting 678-622-0079.
If I’m with a patient and unable to answer, please leave a voicemail and I will return your call as soon as possible.
If you prefer to text, please include:
Your full name
Cell phone number
Email address
Your preferred day and time for your visit
Once I receive your message, I’ll be happy to schedule your appointment and guide you through the next steps.
Step 2: Fill out the paperwork
Once you schedule your first appointment, you will automatically receive a link via email to complete your paperwork online.
If you would like to get a headstart and fill out your forms before scheduling, you may use the link below at any time.
If you prefer to complete your paperwork in the office, please arrive 15-20 minutes before your scheduled visit.
Step 4: You arrive for your appointment… what’s next?
When you arrive at the office at your appointment time, you have already watched the Orientation video and most likely have printed and filled out the new patient forms. If you have not yet fill out the forms, you will be given a chance to do so. The doctor will review your paperwork and perform an evaluation. This usually includes evaluating your posture, feeling your spine and surrounding tissue, and analyzing for subluxations. If the doctor deems it appropriate and safe, you will receive an adjustment.
Step 5: You’re All Set!
The power has been turned on! Go out and enjoy your day! We look forward to seeing you in 7-10 days!
Step 3: Watch the Orientation video
Watching the Orientation videos is mandatory. If you choose not to watch the video, please choose one of the other great chiropractors in your area. Included in the new patient forms is a brief quiz for you to fill out after you watch the video just to make sure you catch all the key points. Don’t worry, it’s super easy if you watched the video! Please be aware that you will not be accepted as a practice member until you watch the Orientation video and complete the quiz.
